• ਸੂਚਨਾ ਫਾਰਮ ਨੰ. 2 ਮ੍ਰਿਤਕ ਵਿਅਕਤੀ ਦੇ ਪਰਿਵਾਰ ਵੱਲੋਂ ਭਰਿਆ ਜਾਵੇ।
• ਮ੍ਰਿਤਕ ਦਾ ਅਸਲ ਆਧਾਰ ਕਾਰਡ ਅਤੇ ਵੋਟਰ ਆਈ.ਡੀ. ਕਾਰਡ।
• ਸੂਚਨਾ ਫਾਰਮ ਅਨੁਸਾਰ ਪਰਿਵਾਰ ਦੇ ਬਾਕੀ ਮੈਂਬਰਾਂ ਦੇ ਅਸਲ ਆਧਾਰ ਕਾਰਡ।
• ਸੂਚਨਾਕਾਰ ਦਾ ਅਸਲ ਆਧਾਰ ਕਾਰਡ 
• ⁠ਮੋਹਤਬਰ ਵਿਅਕਤੀ ਦੇ ਮੋਹਰ ਸਮੇਤ ਹਸਤਾਖਰ 
• ਗਵਾਹ ਦਾ ਅਸਲ ਆਧਾਰ ਕਾਰਡ ਅਤੇ ਸੂਚਨਾ ਫਾਰਮ ਉੱਪਰ ਮੋਬਾਈਲ ਨੰਬਰ ਦਰਜ ਕੀਤਾ 
 ਜਾਵੇ।

ਵਿਸ਼ੇਸ਼ ਨੋਟ:-
ਸੂਚਨਾ ਫਾਰਮ ਨੰਬਰ 2 ਭਰਨ ਸਮੇਂ ਕਿਸੇ ਪ੍ਰਕਾਰ ਦੀ ਕਟਿੰਗ, ਓਵਰਰਾਈਟਿੰਗ ਅਤੇ ਫਲੂਇਡ ਦੀ ਵਰਤੋਂ ਨਾ ਕੀਤੀ ਜਾਵੇ

Required Documents for Registration of Death of a Deceased Person
* Application Form No. 2, duly filled in by the family of the deceased.
*Aadhar and voter id card of the deceased person
*Aadhar card of family members of the deceased.
*Aadhar card of the informer.
*Sign with stamp of MCs/Sarpanch/Panch/Gazetted person.
*Witness’s adhaar card.

Special Note:
*No correction, overwriting, or alteration should be made in Form No. 2 under any circumstances.
*Note down the mobile number of witness on the form number 2
